The Company works with a local occupational physicians group to provide the best
medical care for work related injuries. We strongly recommend that you utilize these
services in the event of an injury as they are very familiar with our organization and the
jobs individuals perform. You may be treated by your own doctor for any job-related
injury if you notify the Company in writing of the name of your personal physician before
you are injured. Forms are available for this purpose. Your “personal physician” means
a licensed physician or surgeon who has treated you in the past and who keeps your
               medical records.

If your work-related injury requires a leave of absence, this leave may count toward your
annual family and medical leave and under the FMLA or CFRA, if you qualify for leave
under those programs. As detailed herein, the Company provides leave in addition to
the FMLA/CFRA and Disability leave for employees who have work-related injuries.

               CRIME AND ROBBERY

If you are ever involved in a robbery, DO NOT RESIST. Statistics show that people,
who resist, are three times more likely to be injured than people who do not resist. The
safety of you, your fellow employees and customers are our highest priority. Don’t be a
hero. Always cooperate fully and do not resist!

               FIRE PROTECTION

All employees must know the specific location and operation of fire protection in the
Restaurant. The Restaurant is equipped with many fire-extinguishing systems in the
ducts, hood, over the stoves and other cooking equipment that contains a dry chemical.
They can be set off immediately by pulling the ring attached to each system. We also
maintain hand held CO² systems that are available throughout the restaurant. Be very
specific before setting off a fire alarm or notifying someone to take action.

If the fire alarm sounds, assist guests to the nearest fire exit and out of the building
immediately. Tell them the restaurant is under “Fire Alarm Status” and it is their
responsibility to leave the restaurant through the nearest exit.

               SMOKING

In accordance with California law, smoking is prohibited in all indoor locations on
Company property and at certain outdoor locations. A designated smoking area is
provided at the back end of the employee parking lot.

               ALCOHOL

Amy’s Drive Thru is a non-alcoholic restaurant and any observation of alcohol on the
premises must be immediately reported to management.
