SRNA provides addi onal Life & Accidental Death & Dismemberment benefit for you while you are traveling on behalf of the
company. The Business Travel Accident Program is an added asset protec on provided at no cost to you! If traveling on company
business, you are protected by this program 24 hours a day, 365 days a year for insured accidents occurring anywhere in the world
(see contract for exclusions and limita ons). Benefits are payable in addi on to any other insurance which may be in effect at the
me of the covered accident.

ANTHEM BLUE CROSS | TRAVEL ASSISTANCE PROGRAM
SRNA provides a Travel Assistance plan to you at no cost. This program, arranged Anthem Blue Cross and provided by Generali
Global Assistance, offers emergency assistance when you and your dependents are traveling 100+ miles away from home on a trip
lasting 90 days or less. Travel Assistance is available 24 hours a day, 7 days a week with trained professionals to provide valuable
support in a medical emergency, such as locating doctors, dentists and facilities, setting up emergency transfers, sending travel
companions, pets, and vehicles home, coordinating bedside visits, and setting up and paying for the return of mortal remains (see
contract for restrictions). In addition, emergency travel support services are available and provide services such as emergency
payment and cash advances, emergency messages, and legal help.

DISABILITY INSURANCE
ANTHEM LIFE | SHORT TERM DISABILITY INSURANCE
SRNA offers you Short Term Disability (STD) to provide income replacement if you become disabled due to accident, sickness or
pregnancy, through Anthem Life. If you experience a temporary disability, benefits begin the day of an accident and 7 days a er
the start of a sickness or pregnancy and will con nue as long as you remain disabled up to a maximum of 13 weeks. STD works with
state disability programs, Social Security, and any other group disability coverage, to provide you with a combined monthly benefit
equal to 67% of your pre‐disability earnings up to a maximum benefit of $2,423 per week.
ANTHEM LIFE | LONG TERM DISABILITY INSURANCE
SRNA offers you Long Term Disability (LTD) to provide income replacement if you become disabled for an extended period of me,
through Anthem Life. If you become totally and permanently disabled, benefits begin 90 days a er the start of your illness or injury
and will con nue as long as you remain disabled up to Social Security Normal Re rement Age. LTD works with state disability
programs, Social Security, and any other group disability coverage, to provide you with a combined monthly benefit equal to 67%
of your pre‐disability earnings up to a maximum benefit of $10,500 per month.
