Our Gym Rebate is Also Offered Through Vitality!

Register for Vitality to start tracking your workouts! After you have completed 100 Standard or Advanced Workouts in the
given plan year, submit your rebate request online and your deposit of $300 will arrive directly in your bank account. It’s
that easy! Please note we are required to tax employees on all Vitality Rewards. This includes the Apple Watch Program,
Gym Rebate Program and Gift Cards redeemed through the Vitality Mall.

Vitality Points
You earn Vitality Points for every Vitality activity completed. Your Points Status resets each year since it is based on the
number of points you have earned in the current year. You can always check-in on your current status within your Vitality
Profile on the Vitality Home Page.
Complete the below required wellness activities to receive a $50 gift card and a $100 Vitality Marketplace
health credit once you hit Silver Status and an additional $100 gift card if you hit Gold Status!

• Complete the Vitality Health Review
• Complete your Vitality Check screenings

Vitality incentives are reported as taxable income and the required tax withholdings will be deducted by DPR from a future
paycheck accordingly. The Vitality program year runs from 9/1/2021 through 8/31/2022. Be sure to submit your request for gift
cards before then. However, you have up to 90 days after the program year ends (8/31/2022) to submit for the gym rebate.
