Recommendations

            1.  TPI has overlooked the ITP criteria and issued IRN.

            2.  The material shall be accepted only after meeting the technical specification of the purchase orders.
            3.  Welding of lifting lugs shall be witnessed & certified by TPIA / OEM as per ITP / QAP and certificate for
                 the same to be submitted along with the supply.
            4.  Dye penetration test to be carried out for lifting lugs before erection.

            5.  OEM shall submit certificate against design load carrying capacity of lifting lugs.

             Incident


            MSQ Penex feed drier DR-203 adsorbent (Drier adsorbent is flammable when exposed to air and starts to burn.)
            unloading was in progress under inert atmosphere by loading/unloading agency. Site supervisor witnessed flash
            fire/smoke  came  out  from  the  top  manhole  of  feed  drier.  Immediately  Nitrogen  flow  inside  the  drier  was
            increased and flash fire/smoke got eliminated.



             Root Cause

            Non compliance of SOP (Inadequate nitrogen blanketing causes adsorbent to come in contact with oxygen).



             Recommendations

            1.  Nitrogen blanketing to be continuously ensured during unloading. Any increase/decrease of nitrogen flow
              to be done under operations supervision.
            2.  Before start of unloading, portable O2 meter reading should be NIL at reactor/drier top cover for
              adequacy checking of nitrogen blanketing.

            3.  Nitrogen blanketing to be ensured by analyzing the O/L sample and Checking of O2 & N2 %. In the same
              O2 % in O/L should be Nil and N2 should be +ve side.
            4.  SOP must be prepared and circulated to all concerned and having approval from competent authority.
              The same must be available in the operating manual also.





             Case 18                              Date: 16.02.2020                      Time: 09:45 hrs.


             Incident

            Two workers were working on AFC platform above the main pipe rack in BS-VI DHDT project site at an elevation

            of 22 Mtrs. While taking measurement, the right angle measurement tool inadvertently slipped from the hand of
            a worker & fell though the gap in the floor grating. The right angle collided with number of pipes & structural
            components before hitting the shoulder by brushing through Sikh turban worn by the contractor worker.
