Root Cause
EOT movement limit restriction towards Extreme West side of plat former Compressor.
Recommendations
1. For roller assembly metallurgy & strength related issues OEM to be consulted
2. JSA to be done in view of EOT travel restriction and space constraint and Barrel shifting, sling adjustment
related points to be incorporated in JSA with all potential possible hazards.
Case 25 Date: 15.05.2019 Time: 08:00 hrs.
Incident
A casual of inspection department opened the main door of the inspection building and found that the
entrance area was smoky. Immediately Fire & Safety team reached the site and found that towards just right
to the entrance of inspection building, electrical wirings are hanging, sofa found totally burnt, One wall
mounted fan found burnt, tube light found burnt and one TV ahead to lobby is found burnt. Most of the walls
and roof towards right inside lobby found blackish with smoke.

Root Cause
Jamming of wall mounted fan shaft resulted in generation of heat and melting of plastic components and
subsequently caused fire. The thermostat present near the fan motor portion did not act and hence, the
power supply was not cut off.
