Recommendations
1. Adequate sloping / shoring / shielding to be ensured during excavation to prevent collapse of
excavation walls.
2. Excavated earth should be placed at safe distance (atleast one meter away) from the edge of the
excavation pit.
3. Ensure hard barricading the site to prevent vehicles from operating near the excavation sites at least
4 to 5 m from the edges of excavation.
4. Additional precautions to be taken against water ingress which may result in collapse of side walls.
Excavation should be avoided during rainy season.
5. Ensure that water is not flushed near the excavation site from adjacent areas during the flushing activities.
All necessary precautions as envisaged in OISD-192-Safety Practices During Construction, Cl.No. 6.1 shall
be ensured at site.

Incident
H2 Gas detector alarm was noticed in DCS panel, On intimation SIC / EA visited the site and observed that
there was high CO (1000 ppm) level in Multi Gas meter at MT/LT Reactor top PSV platform. Subsequently
leak was identified from weld joint of 6” spool piece of PSV-3658 (E-204 downstream PSV, Service: Process
gas having composition H2, CO, CO2, Ch4). PSV was immediately isolated, joint site visit done by PN, MM
and Inspection dept.

Root Cause
1. Weld joint failure from 6” spool piece of PSV-3658.
2. Material defect in 6” weld neck flange.
