Recommendations

1. Insulation of MP steam header to be replaced.

2. Soil under 4” interface header (DHDT feed component) & soil under LPDs in the same line need to be
removed, sufficient concreted ground clearance (300mm) to be maintained to avoid any corrosion from

soil/water logging. A channel to be made so as to allow of rain water etc. from below the ground level
piperack towards the safe location.

3. Clamped line to be replaced with NB 4" ASTM A106 GR B, standard schedule pipe from upstream elbow
to downstream elbow.
4. Survey to be done for identification of all such HC lines for all OC-3/ OC-4 area, having low ground

clearance. Corrective action to be taken thereof.
5. Periodic removal of oil from the trenches, cleaning & desilting of trenches on quarterly basis needs to

be ensured, especially in offsite pump house area.

Incident

Around 4m high vertical excavation wall at the incinerator area collapsed while no worker was inside the
excavated pit. Civil jobs were in progress at the excavated pit and workers were engaged in removing the
shuttering boards etc. during previous days.

Root Cause

1. Collapse of soil due to water seepage from adjacent area on account of flushing activities.
2. Inadequate protection system in place (Excavated portion was not latterly supported by
shoring/ shuttering).
