Case 23                              Date: 02.03.2020                      Time: 11:45 hrs.



             Incident

            While preparation for Cover box up of MCC Package Condenser No.1, North side channel head cover (weighing
            around 0.8T) of the condenser fell down on the floor gratings due to failure of the weld joint of the sliding link

            hanging the channel cover on davit arm. Gratings and minor beam below the condenser got damaged due to the
            fall of channel head cover.


             Root Cause

            Failure of weld joint of the sliding link on davit due to the defective material / weld joint of sliding link.


             Recommendations


            An SOP shall be developed incorporating the various safety measures to be taken before opening of such
            covers having davit arm assembly. This SOP shall also include the various checks wrt to the visual conditions
            like corrosion, visible cracks, dislodging, insufficient welding etc. in the davit arm assembly. In case, these are
            not found to be in good condition, these are to be replaced / rectified and tested before putting into use.

            This SOP will be applicable to existing as well as new units.




             Case 24                              Date: 12.09.2019                      Time: 12:25 hrs.


             Incident

            Compressor barrel was to be dismantled and pulled out for its overhauling & maintenance job. Barrel was being
            pulled out and it was in position on the guide rail. While fixing of lifting gears it was observed that the EOT

            crane was not reaching the Centre of Gravity of barrel and while shifting of lifting gear (web sling) suddenly
            guide rail bolt got sheared off letting the barrel part to settle over the rail. Due to this left foot of supervisor
            caught in between the rail and barrel reslted in injury in his littler finger.
