Root Cause

            1.  Improper securing of tools by worker while working at height.

            2.  Inadequate safety net / barricading.


             Recommendations

            1.  Workers to properly secure tools while working at height.
            2.  Dislodged gratings shall be properly fixed without any gaps.

            3.  Restricted commutation below pipe rack, which is multitier working platform.
            4.  Co-ordination among various contractors for multitier working.
            5.  Adequate Safety net / barricading covering the whole work area to be ensured.





             Case 19                              Date: 20.02.2020                      Time: 08:30 hrs.



             Incident

            On 20.02.2020 at 08.30 Hrs, one Vehicle (Mahindra Bolero) Of contractor, while going from ETP-1 to Storm water
            pond, accidently collided with Electrical Light Pole on Road-D, at south side of TPS C/R resulted.


































             Root Cause

            (Negligent driving) Driver lost control of Vehicle: use of accelerator in place of brakes.



             Recommendations

            1.  Drivers to be sensitized about the incident and cautioned to follow speed limit inside the battery area.
            2.  Drivers should not work for more than 16 hr inside refinery. ACS system to be implemented.
