Root Cause

            1.  Cooling water return valve got stuck in closed position resulted in reduced CW flow through ejector

                 condenser and positive pressure in vacuum column.
            2.  Leakage of slop oil from an existing tack weld joint and resulted in Fire due to auto-ignition.


             Recommendations


            1.   Leaky point of the line to be clamped and online Sealed.
            2.   Scheme for cooling water bypass line shall be implemented (across stuck open return valve) for smooth
                 back-flushing of the condensers.

            3.   SOP / JSA to be followed during back-flushing activity.





             Case 33                              Date: 29.09.2020                      Time: 23:17 hrs.


             Incident

            On call from MCP-186 (Road-B, near fabrication yard.) received at main fire station control room. Fire crew
            rushed to the site and found sparking/ electrical flashover in underground LT power cable at two different
            locations. UCR was informed to cut off power supply of fabrication yard. After that power supply was
            isolated from SS#9 by UCR personnel and situation was brought into control.

             Root Cause

            1.  Short circuit leading to flashover due to cable damage at road crossing.
            2.  Deviation from procedure. Laying of power cable was not in accordance with standard practice.
