         4. Guidance on Changing Plans



         While you should carefully choose your managed care plan initially, you do have the option to
         change plans if your current plan is not meeting your needs.



         When You Can Change Plans
             Initial  Enrollment  Period:  When  you  first  enroll,  you  typically  have  120  days  to  change
             plans without needing a reason.
             Open Enrollment: Florida has designated open enrollment periods when you can switch

             plans (similar to health insurance open enrollment periods).
             For Cause: You may be able to change plans outside these periods for specific reasons
             such as moving to a new area or if your current plan is not meeting your needs.



         How to Change Plans
            1.Online: Log into the FL Medicaid Member Portal at www.flmedicaidmanagedcare.com
           2.By Phone: Call Choice Counseling at (877) 711-3662
           3.By Mail: Complete and submit a plan change request form



         Before Changing Plans: Verify that your current providers (facility, doctors, specialists) are in
         the new plan’s network. Also confirm that any services you are currently receiving will continue
         to be covered.
