HSSE REQUIREMENT FOR CONTRACTOR, ALLO TECHNOLOGY

                                 Workers exposed to traffic hazards while working on the road or at worksite shall be
                                 provided with a reflective vest made of highly visible materials supplemented with
                                 reflective material visible on all sides of the wearer.

                              b.  Protective clothing
                                 Workers  shall  be  provided  with  suitable  protective  clothing  such  as  waterproof
                                 protective suit, camouflage bee prove face mask net, hood, apron, leggings, gloves
                                 and  other  protective  wear  suitable  to  the  nature  of  the  substances  and  the  risks
                                 involved.


               4.16.      Permit to Work (PtW)
                         I.   The  Contractor  shall  comply  to  all  relevant  PTW  requirement  by  Allo,  TNB  and  local
                              authority/3rd party issuer. Contractor shall nominate a competent person for the purpose
                              of receiving PTW from authorized personnel of the premise.
                         II.   Contractor shall pose valid PTW at all time for the duration of the Works and shall be
                              produced whenever requested by Allo or TNB Personnel.
                        III.   When required, contractor shall visibly display the “Permit to Work” at the Works site
                              throughout the duration of the Works and shall be returned to the authorized personnel
                              after the job has been completed.
                        IV.   All high-risk activities shall use PTW system. The Employer reserves the right to impose
                              PTWs  to any construction activities  that deem to him/her is a high-risk  construction
                              activity. Life critical / high risk activities are:
                              a.  Working at heights
                              b.  Lifting
                              c.  Confined space
                              d.  Hot works
                              e.  Deep Excavation
                        V.    The Employer reserves the rights to stop any work under this category that is underway
                              without proper contractor permit in place and the Contractor shall have no recourse to
                              claim for any extension of time as a result of work stoppage.

               4.17.      Emergency Response Preparedness (ERP)
                      I.   The Contractor shall establish a written Emergency Response Plan to ensure all personnel are
                          safe from fire and other emergencies.
                     II.   The plan shall be reviewed and communicated to all employees and ensure related training
                          before  implementing  the  Emergency  Response  Plan  to  sufficient  number  of  workers  at
                          worksite.

               4.18.      Notices and Site Safety Signage
                   a)     The Contractor shall install and maintain on site approved standard safety signage and notices
                          to provide an effective means of communicating information concerning the hazards to the
                          employees or any persons at worksite. The contractor must put proper safety signage at all life
                          critical / high risk activities.
