1-413   Machine guards- are provided for your protection from entanglement
                                      from moving parts or machinery.  Guards should be properly
                                      designed, installed, secured, and maintained.

                              1-414   Guards must be properly in place before operating equipment or
                                      machinery.  Never place your hands or other body parts near moving
                                      machine parts while lubricating (oil-grease) machinery, washing mixer
                                      drums or in other areas where there is a danger of being caught
                                      between moving machine parts.

                              1-415   Fall Prevention  requires you to recognize potential slip, trip or fall
                                      hazards in your work environment.  When working from elevation,
                                      where there is a potential for a fall, either fall prevention systems
                                      and/or a personal fall protection system is required.   Fall prevention
                                      systems include, work platform and walkways with proper handrails,
                                      aerial lift etc.  Fall protection systems include body harness, lifeline
                                      (retractable or fall limiting lanyard) and proper anchor point.  You must
                                      maintain three points of contact when climbing ladders, and
                                      entering/exiting mobile equipment.  Always work at a safe, productive
                                      pace and keep your eyes on your path.

                              1-416   Confined  Space hazards exist where entry and/or exit of persons are
                                      limited and the space is not intended for permanent occupancy.
                                      These are areas where walls converge such that entry or exit is
                                      limited, areas where the atmosphere could create oxygen deficiency,
                                      respirable exposures above a PEL (Permissible Exposure Limit) or
                                      IDLH (Immediately Dangerous to Life or Health), flammable or
                                      explosive, engulfment or other physical hazards to entrants.
                                      Examples include, but are not limited to:  bins, hoppers, silos, mixer
                                      drums, dryers, water truck tanks, electrical vaults, barge hulls, etc.
                                      Specific training, practices (procedures and/or permits) and DANGER
                                      – PERMIT REQUIRED CONFINED SPACE – DO NOT ENTER signs
                                      apply.  Supervisors, entrants and attendants must be knowledgeable
                                      of the potential hazards, duties, and rescue plans before allowing
                                      person(s) to enter and conduct work in a confined space.

                              1-417   Before repair-maintenance-construction activities, conduct a
                                      hazard analysis (TAKE 2, prejob meeting).  Conduct a work plan
                                      for all tasks and/or jobs that involve three or more personnel, a crane
                                      or a lifting device or infrequently performed jobs/tasks. Obey signage
                                      and all audible and visible warning devices, hand signals (ANSI) and
                                      physical barricades.  Never stand under suspended loads. Avoid
                                      areas where there is potential for falling objects or moving equipment,
                                      such as the swing radius of a crane. Non-conductive tag lines help
                                      control safe movement of a suspended load.

                              1-418   Good  housekeeping practices in your work area must be maintained.
                                      Your work area should be free of slip, trip and fall hazards.  Tools,
                                      materials and debris should be placed in a manner that does not
                                      hinder safe travel or access. Take pride and ownership in your
                                      workplace.
