                                                         Commentary:
                                                         Two recordables occurred with Enterprise, and Two Cate-
                                                         gory 4 incidents as shown in the Pyramid.
                                                         A Reminder of changes made to this scorecard:

                                                         •  Addition of the workflow status for incident investiga-
                                                            tions (report only, verification, investigation, approval,
                                                            closed).

                                                         •  Consolidation of the interactions metrics into one for
                                                            LMW (field/floor interactions).

                                                         •  Removal of the body injury graphic.




                Recordable RV7: Fractured finger, hit hand with sledgehammer installing wheel chocks.

                Recordable CRG1: Fractured finger, hit hand with sledgehammer installing wheel chocks.
                LCD OTS: Water truck driver attempted to extinguish a smoker under stopped train on live track.

                RGS5: Operator moved machine without instruction from the EIC.
                Hamel Shop: Knee hit by suspended load during movement.  Report Only.

                Hamel Shop: Grind dust in eyes, first aid.
                Hamel Shop: Wasp sting to the nose, first aid.

                DC6: Ankle strain after stepping off the hopper onto a rock on the platform, first aid.
                C2104: Shoulder strain while stacking stones in the stone room, first aid.

                Specialty Maintenance: Rear ended by third party vehicle. Damage.
