The employee may retain any mileage allowance paid by the court for jury or witness
                 services.


                 DISABILITY LEAVE BENEFITS
                 Eligibility


                 Full time employees, working on a scheduled normal workweek of at least 30 hours per
                 week may be eligible for disability leave benefits.   For additional information on eligibility
                 see the summary plan description.

                 SHORT TERM DISABILITY

                 If the employee anticipates that a medical disability would prevent the employee from
                 working he/she may apply for short-term disability benefits through the Jones-Hamilton
                 Co. disability provider.  Eligible employees will receive a Benefits Summary Guide for
                 complete details.


                 Disability leave will run concurrently with any FMLA leave to which the employee is
                 entitled.


                 Employees will not accrue PTO, sick time, holidays, etc., during short term disability and
                 will not be allowed to use sick time or PTO to offset wages after the elimination period.

                 LONG TERM DISABILITY


                 If the employee anticipates that a medical disability would prevent the employee from
                 working and will exceed ninety (90) days in length, he/she may apply for long-term
                 disability benefits. Eligible employees will receive a Benefits Summary Guide for complete
                 details.

                 Employees will not accrue Paid Time Off, sick time, holidays, etc., during long term
                 disability.
