                 WORKERS’ COMPENSATION

                 All employees are covered by workers’ compensation insurance, which compensates an
                 employee for lost time, medical expenses, and loss of life or dismemberment from an
                 injury arising out of or in the course of work.
                 To ensure that you receive any workers’ compensation benefits to which you may be
                 entitled under applicable law, you will need to:
                     ▪  Immediately report any work-related injury to your supervisor;
                     ▪  Seek medical treatment and follow-up care if required;
                     ▪ Review that supervisor’s written Incident Report (part 1 & 2) and return it to
                        their immediate supervisor, or Human Resources.

                 Upon submission of a medical certification that an employee is able to return to work
                 after a workers’ compensation leave, the employee may be reinstated to his or her
                 same position held at the time the leave began, or to an equivalent position, if
                 available. However, changes in assignments, duties, hours, and schedules may occur
                 out of business necessity.

                 An employee’s return from workers’ compensation leave depends upon Jones-Hamilton
                 Co.’s needs, the duration of leave, and the employee’s qualifications for existing
                 openings, among other factors. On return, the employee will have no greater rights
                 than if he or she had not been on leave. An employee on workers’ compensation leave
                 is not protected from actions that would have affected the employee if he or she was
                 not on such leave (e.g., shift eliminated).

                 Temporary Modified Duty
                 When possible, Jones-Hamilton Co. will provide modified work for employees who have
                 been restricted by a health care provider due to a work-related injury or illness.
                 Modified duty will be provided subject to availability and will be assigned according to
                 the nature of the injury or illness and the limitations set forth by the treating health
                 care provider.

                 Employees on temporary modified duty must furnish a written update of their medical
                 condition to Human Resources from the treating health care provider after each visit in
                 order to remain on modified duty. Temporary modified duty assignments are limited to
                 a period of 90 days, subject to review.

                 MANAGED CARE ORGANIZATION
                 Jones-Hamilton Co. provides medical treatment for work-related injuries through
