➢  If  the  qualifying  exigency  is  to  spend  time  with  a  military  member  on  Rest  and
                       Recuperation leave, employees must submit a copy of the military member’s Rest and
                       Recuperation orders or other documentation confirming the dates of the military member’s
                       leave.

               MEDICAL LEAVE OF ABSENCE

               This policy applies to all employees of Mission who are not eligible for FMLA leave or other state-
               mandated medical leave. A Medical Leave of Absence is intended to allow an employee to be
               absent from work for his/her own medical condition. A Medical Leave of Absence may be taken
               consecutively, intermittently or in the form of a reduced work schedule if medically necessary.
               Employees may participate on the first day of the month after completing the 6-month waiting
               period.  Upon approval, a Medical Leave of Absence may be granted for up to a total of twelve
               (12) weeks in a 12-month rolling backward period. Any extension requests beyond twelve (12)
               total weeks will be reviewed for a reasonable accommodation under the ADA and similar laws.
               Employees, however, are reminded that they must provide sufficient information to Mission so
               that it can make such a determination. Requests that pose an undue hardship or are indefinite in
               nature  generally  will  not  be  granted.  For  further  information,  please  contact

               SICK LEAVE

               Some state or local laws mandate employers to provide sick leave to their employees. For more
               information about the granting of sick leave, please contact mpcpayroll@missionpharmacal.com.

               PERSONAL LEAVE POLICY

               This policy applies to all full-time employees of Mission. The Personal Leave Policy is intended
               to provide unpaid time off in special or rare circumstances where an employee may need to attend
               to personal or family affairs not covered by any other leave provided by Mission.  Generally, these
               leave requests will be evaluated based on the reason for the leave request, anticipated workload
               requirements, staffing considerations during the proposed period of absence, and other business
               concerns. During the Personal Leave period, the employee is responsible for his/her portion of the
               regular group health insurance premium (if applicable) of such employee.

               Personal Leave is non-statutory leave; therefore, every effort will be made to return the employee
               to the same position, if available, or to a similar available position for which the employee is
               qualified. However, Mission does not guarantee reinstatement upon return from a Personal Leave
               of absence from work.
