copy of the Military Leave Order Request form. If the leave period deviates from the original leave
               orders,  then  employees  are  encouraged  to  provide  any  documents  for  the  deviation.  Eligible
               employees may use any accrued, but unused PTO time for their absences.   For further information
               on eligibility, employment rights while on leave and job restoration upon completion of leave,
               please contact the People Operations Department.

               JURY DUTY

               All eligible employees will be provided with the necessary time off to serve on jury duty. While
               serving on jury duty, full-time employees will receive wages up to two (2) weeks per calendar
               year. For example, if an employee is regularly scheduled to work thirty-seven and one-half (37.5)
               hours per week, he/she will receive wages up to seventy-five (75) hours. If an employee is regularly
               scheduled to work forty (40) hours per week, he/she will receive wages up to eighty (80) hours.
               Part-time employees will be eligible to receive up to one (1) week of jury duty leave, based on the
               regularly  scheduled  workweek.  When  an  employee  receives  a  jury  duty  summons,  then  for
               scheduling purposes the employee must immediately provide a copy of such jury duty summons
               to     the    employee’s     immediate      supervisor/manager     and     to    payroll    at
               mpcpayroll@missionpharmacal.com.

               In addition, written proof of service (jury duty certification) with a date stamp is required from the
               court  for  payment  of  jury  duty  leave.  This  proof  of  service  must  be  submitted  to
               mpcpayroll@missionpharmacal.com immediately following return from jury duty.

               Absences of greater than two (2) weeks will not be paid. Employees may use PTO for unpaid jury
               duty time.

               SUBPOENA OR NOTICE TO APPEAR

               If  an  employee  is  personally  served  with  a  subpoena  or  receives  a  notice  to  appear  in  any
               administrative or judicial proceeding, then such employee will be provided with unpaid time off
               work. The employee must provide his/her immediate supervisor/manager with as much notice as
               possible and a copy of his/her subpoena or notice to appear.  The employee’s PTO will be utilized
               for this leave, if available. Once PTO time has been exhausted or if no PTO is available, additional
               time off will be unpaid.
