after the member became a veteran. The treatment, recuperation, or therapy must be obtained
within five years of the member becoming a veteran.
Documentation for Family Leave
Upon request, Human Resources will furnish you the necessary FMLA forms which need to be filled out
and returned to Human Resources. The forms will include a letter explaining your rights and
responsibilities under GreenPoint Ag’s leave policies.
If you are absent for Family Leave, you will be required to provide appropriate certification before the
leave begins, periodically throughout the leave, and/or when you return to work. Failure to return to
work upon the expiration of a leave of absence or refusing an offer of reinstatement for which you are
qualified, will result in voluntary termination.
Medical Leave
FMLA leave may be used when an eligible employee is on medical leave for his or her own serious
medical condition.
Eligibility for Medical Leave
If you have worked for GreenPoint Ag for at least twelve months and have at least 1,250 hours of service
during the previous 12-month period, you are entitled to request FMLA leave up to twelve weeks for
your own serious medical condition, which renders you incapable of performing the essential
requirements of your position.
Intermittent and Reduced-Hour Leaves
Intermittent or reduced-hour leave under the Medical Leave will be permitted only if it is for your own
serious health condition or for your immediate family member (child, spouse, or parent) experiencing a
serious health condition.
Documentation for Medical Leave
If the leave is being designated as medical leave, Human Resources will furnish you the necessary forms,
including a letter outlining your rights and responsibilities under FMLA.
If you are absent for medical leave, you will need to provide certification from a health care provider
before medical leave begins, periodically throughout the leave period, and/or when you return to work.
Under certain circumstances, GreenPoint Ag may require, as a condition of continued employment,
prior to or during medical leave, that you be examined at GreenPoint Ag’s expense by a health care
provider GreenPoint Ag selects to furnish a second opinion and, if appropriate, a third opinion.
While on a medical leave, you need to contact Human Resources and your manager at least every thirty
(30) days and provide updates on your medical condition. Failure to do so may result in what would be
considered a voluntary termination. Failure to return to work upon the expiration of a medical leave of
absence or refusing an offer of reinstatement for which you are qualified, will result in voluntary
termination.
