Do you have any medical background or
family history?
Nurse: Sir, since your patient is a bit agitated, could you help me answer some questions for her
while the doctor examines on her?
Director Choi: Sure, why not!
Nurse: What is the full name of the patient and your relation to her, Sir?
Director Choi: My wife’s name is Olivia Choi.
Nurse: What is your exact address?
Director Choi: We’re from Korea and we’re just here for a business trip.
Nurse: I got that, Sir but where do you stay here in New York?
Director Choi: We have a hotel reservation at InterContinental Hotel, New York.
Nurse: Oh is that so, Sir? How old is she and how many children do you have?
Director Choi: She is 32 years old, and we do not have any children.
Nurse: Did she suffer from any injury or has undergone any medical treatment and/or any family
history or medical background like Asthma, Diabetes or Tuberculosis?
Director Choi: No Madam, she hasn’t experienced any; hence, we don’t have a family history or
a medical background.
Nurse: Do you have a Medical Card or Health Insurance Card; if you do, what’s your card num-
ber?
Director Choi: Her International Medi-card number is 31908650.
Nurse: Thanks so much. Kindly sign this form on her behalf.
1. Do you think that it’s necessary to know your family history and medical
background?
Sample answer:
Yes, everybody should be aware of his/her family history or medical background for
personal protection and reference. If we know our family history, we may be able to
avoid the things, food, habits, lifestyle and pathogens that could affect our body easily
thus we can be safe from being infected with such disease.
2. Why is your personal information paramount in the hospital?
Sample answer:
The patient’s personal information is necessary in a hospital because this serves as the primary
identification on each client. With proper data encoded, the health practitioners could easily
identify one patient from the other to give a more organized means of diagnosis and treatment.
