If this form is not submitted and approved by the School Dean, the student risks having to start their
program over, thus losing all credits and coursework already completed.
Readmission
If a student has been out of the program for one (1) or more years, the student must be readmitted
into the graduate program. The student must seek advisement from the Admissions office, their
former Advisor, and/or the Department Chair and finally complete a Graduate School Application
for Readmission (see Appendix A: Forms) to be readmitted, reactivated as an official student.
Plagiarism/Academic Honesty
As stated in the Graduate Catalog, plagiarism is the act of representing another’s ideas, words, or
information as one’s own. Students guilty of plagiarism are subject to severe penalties ranging from
failure for the assignment to failure in the course or, in extreme cases, dismissal from the University.
