COLLEGE POLICIES
Admission Guidelines & Procedures for Level I
The College of Nursing implements an open admission and selective retention policy.
Applicants to the Nursing Program must:
1. Have completed the high school program and belong to the upper 40% of the graduating class.
2. Submit the following entrance documents to the Screening Committee:
a. Form 138 or High School Report Card, the General Weighted average should be 85% or
above
b. Entrance examination result issued by the Guidance Office, which should be 40% or above.
c. Certification issued by the High School Principal/Registrar that the applicant belongs to the
Upper 40% of his/her graduating class in high school.
d. Certificate of Good Moral Character issued by the High School Principal or Guidance
Counselor.
e. Two 2x2 recent ID picture (colored)
f. Certification of physical examination conducted by the University Physician
g. Birth certificate from PSA / authenticated by PSA
h. Marriage certificate (if applicable)
3. Pass the interview to be conducted by the Nursing Screening Committee.
4. The Screening Committee of the College sets a schedule of screening each year. Screening
includes submission of requirements, ranking of applicants, posting of qualified applicants who are
issued Admission Slips.
5. An applicant who is found physically and mentally unfit as declared by the University Physician or his
authorized representative, even if he qualifies in the other screening criteria, will not be admitted.
The applicant who is next in rank will be considered
6. Only applicants who have obtained the admission requirements shall undergo physical examination
to be conducted by the University Physician.
a. Medical Examination includes the following laboratory works:
Urinalysis
Fecalysis
HBS Ag
Chest X-ray
Complete Blood Count
Drug Test
