and objectives that will be reviewed in a future performance appraisal.  Employees will be given a
             copy of any performance appraisals.

             Questions regarding performance appraisals, general or specific, should be directed to an Employee’s
             immediate supervisor for clarification and/or the Executive Director.

             Insubordination


             All LSBA Employees are expected  to interact with one another  with mutual respect and common
             courtesy.  Employees are expected to take instruction and complete assigned job tasks from their
             supervisors or the Executive Director.  Failure to comply with, or unreasonable delay in completing,
             directives or assigned tasks may be considered insubordination.  Acts of insubordination are grounds
             for disciplinary action, up to and including termination.

             2.15   PERSONNEL FILES AND RECORDS

             The LSBA and each Employee have a shared responsibility to keep personnel records accurate and up-
             to-date to enable the LSBA to reach a family member in case of emergency, to properly maintain
             insurance and health benefits, and to accurately compute payroll deductions.  It is the Employee’s
             responsibility to notify the Executive Director within five calendar days of any change in marital status
             and/or personal status related to:

                          Employee’s legal name
                          Address or telephone number(s)
                          Marital status
                          Beneficiaries or dependents listed for insurance and retirement benefits
                          Number of dependents for tax withholding purposes
                          Person(s) to notify in case of accident or emergency

             The LSBA will timely input updated Employee information into the system for payroll and benefit
             purposes.

             All Employee files and records maintained by the LSBA are the property of the LSBA at all times.
             Employee files and records are confidential and access to such files or records is limited.

             An Employee may request a review of his/her personnel file, in accordance with the following
             guidelines:

                          Submission of a written request to the Executive Director.
                          Upon receipt of such a request, the  Executive Director will schedule an
                           appointment with the Employee to review and/or discuss the Employee’s
                           personnel file or record.
