Consent
Before beginning the survey, obtain consent from the consumer to conduct the survey. Verbal
as well as non-verbal consent such as gestures (written communication, head nod, thumbs up,
etc.) is acceptable. Additionally, you can infer consent in the absence of gestures, behaviors or
any expression indicating otherwise. If consent is provided, check the “Yes” box on ODESA.
However, if the consumer’s exhibited behaviors appear that he/she does not want you present
(ex, pushing the device away, screaming when you talk, shaking his/her head), this should be
accepted as consent not being given and you should not conduct the survey. You should check
the “No” box on ODESA.
When a survey cannot be completed update the survey status on ODESA (SC-2 and SC-3).
Background Information
You may use family members or staff to answer Background Information (BI). Remind the
respondent that they are encouraged to look at any client documents such as the IPP, health
information, and pay stubs for more accurate answers. NOTE: As a surveyor you are not
authorized to view any documents yourself.
