                                                          DGR MANAGER IN TRAINING GUIDE


                Employees are eligible for one personal leave (maximum of 45 days) in any rolling 12-month period.

               To be eligible for personal leave, an employee must:
                   •  Be employed by GESMN for at least 90 days
                   •  Not have received corrective action based on performance within the previous 90 days
                   •  Be a Full-time status employee



               MILITARY LEAVE

               To determine if an employee qualifies for a Military Leave of Absence, please refer to the Employee
               Handbook or have the employee contact The Standard.



               HOW TO APPLY FOR A LEAVE OF ABSENCE OR REPORT ABSENCES WITH THE STANDARD

               When an employee requests a leave of absence, they should contact The Standard as follows:
               Call the Absence Management Service Center (AMSC) at 866-756-8116, available Monday through
               Friday, between 7:00 AM and 7:00 PM Central Time.
               They will be asked to provide the following information – in addition to other questions about their
               absence:
                   •  Employer Name: Goodwill-Easter Seals Minnesota
                   •  Group Policy Number: 752972
                   •  Employee ID number or Social Security number
                   •  Last day at work
                   •  Reason leave is requested
                   •  Physician’s contact information (name, address, phone and fax number)

               The Standard will make and communicate the Eligibility determination within 5 days.  The leave
               determination – approval or denial - can take up to 1-2 weeks to be completed. The Standard notifies
               the Leave of Absence Admin about the determination and the LOA Admin will guide Managers through
               the LOA process.

               WHAT IF AN EMPLOYEE NEEDS AN ACCOMMODATION UPON RETURN FROM LEAVE?

               If an employee has work restrictions upon their return from leave, they should send their restrictions
               documentation to Benefits.

               WHAT HAPPENS IF AN EMPLOYEE IS INELIGIBLE FOR FMLA AND NEEDS TO TAKE A LEAVE THAT
               IS LONGER THAN 45 DAYS?

               The Manager and/or Employee should contact Benefits.
