                                            When benefit coverage begins
                                            For new hires, the following benefits begin on your date of hire. For benefit
                                            changes, the benefits begin on the benefit event effective date, providing you
                                            elect coverage during and enrollment period and meet the plan’s eligibility
                                            requirements.

                                                ■    Dental plan
                                                ■    LiveWell wellness program
                                                ■    Section 125 and flexible spending accounts
                                            The following benefits automatically begin on your first day of employment:

                                                ■    Employee assistance program
                                                ■    Cancer resource benefit
                                                ■    2nd medical opinion service
                                                ■    Group life and accident insurance
                                                ■    Long-term disability insurance
                                                ■    Student loan resource
                                                ■    Workers’ compensation benefits
                                            Coverage begins for other benefits as listed below:

             Short-Term Disability Benefit                   90 days after date of hire
             Supplemental Long-Term Disability Insurance     Date the policy is issued

             Identity Theft Protection                       First of the month following enrollment
