TERMINATION OF INSURANCE
Your insurance will terminate when an insurance policy terminates with a specified carrier, when you fail to make an
agreed contribution to premium when due, or when you cease to be eligible for coverage under the terms of our group
insurance program.
5.3 WORKERS COMPENSATION INSURANCE
To provide for payment of your medical expenses and for partial salary continuation in the event of a work-related acci-
dent or illness, you are covered by worker’s compensation insurance. The amount of benefits payable and the duration
of payment will be determined by the third-party insurance carrier.
