Employees will be responsible for submitting official documentation to payroll for payment. You must notify your Su-
pervisor, in writing within forty-eight (48) hours of receipt of the jury summons. On any day or half-day you are not
required to serve, you will be expected to return to work.
4.7 Vacations Time
All full time employees are eligible for vacation after 90 days of employment. You will begin to accrue as of your first
pay period that you are employed. After your initial 90 days of employment you are able to utilize any accrued time.
Vacation is extended as follows:

Additionally, our policy does not allow vacation time to rollover from year to year. Vacation time will be granted in in-
crements of two (2) hours
Accrued, but unused vacation will not be paid out to terminated employees for the following reasons:
□ Violation of company policies and procedures
□ Job abandonment
□ Failure to return from leave of absence
□ Failure to complete first year of service
□ Accrued but unused vacation will also not be paid to employee’s who do not complete two weeks notice.
Vacation requests must be submitted to your manager at least two weeks before the date in which you wish to begin your
time away. If business operations are in high demand with regards to your responsibilities during a particular time peri-
od, you may be asked to reschedule your vacation for a more convenient time. Vacation will be granted in increments of
two hours.
Vacation time must be exhausted prior to taking time off without pay. Taking time off without pay must also be ap-
proved by your manager.
*When you have exhausted your paid vacation time allotted by the company, any days missed from work thereafter will
be considered unexcused absences unless there is supporting documentation. SEE Section 6.1 for further information.
4.8 Sick Leave/Personal Leave
Full-time and Part-time employees are eligible for sick time after completing the 90 day introductory period. Full time
employees receive 40 hours per year and part time employees will receive 20 hours per year. Sick time will not be car-
ried over from year to year. Sick time will be granted in increments of two (2) hours. Sick time is not to be utilized as
pre-planned vacation time.
