New Matter Intake
Type the number as a 10 digit string and the field will automatically format the phone number
(888.888.8888).
Has Been Audited?
• If the box is checked, the Matter Audit Group has reviewed the matter, updated the client information and the
Retention Agreement should print correctly.
• If the box is not checked, the Retention Agreement will likely need edits.
Billing Instructions
• Billing instructions populate based upon the client. (Not the Carrier or Party Client Represents fields).
• If a client has specific instructions, the information will populate in the field.
• These do NOT print on the Retention Agreement.
Matter Description
• This field will show on the invoice and needs to be descriptive yet brief for the client to know to which matter
the invoice pertains.
• Include the party the client represents and potentially a driver/claimant name as shown in the example below
for L Brands, Ingersol Rand, and UPS.
• Type the Party name first followed by the type of investigation.
• If there are two party names (ie. Cummins / John Doe) or two types of investigation (Fire / Electrical
Investigation), use a forward slash to separate the names and/or words.
o No hyphens, use a forward slash instead (/).
o Use a space before and after the slash.
• Capitalize the first letter of each word.
• Examples:
o Smith v. Jones Biomechanical Analysis (note: no forward slash was used in this example)
o Martinichio Enterprises, Inc. / Binghamton Honda Accident Analysis
o Capital Square Realty Advisors Fire Investigation (note: no forward slash was used in this example)
o L Brands / Aria James Candle Evaluation
o Honda VIMF Testing
o Hess Dwelling Fire / Electrical Investigation
o Ingersol Rand / Marlise Calvo HVAC Evaluation
o "UPS Fire Investigation" is not sufficient information for a client as they could have several UPS cases.
The driver's name would need to be included (i.e. UPS / Joe Moore Accident Analysis).
