New Matter Intake



        Date of Loss
            •  This field will show on the invoice (if populated).
            •  Enter the date in various formats and the field will auto correct.
                   o  04012016 will default to 4/1/2016
                   o  040116 will default to 4/1/2016
                   o  04/01/16 will default to 4/1/2016
            •  A calendar button is available to select the date; the field will auto populate once selected.
            •  Note:  this is not a required field as some matters such as VIMF and Candle testing will not have a Date of Loss.
               However, if submitting a case outside of these types, the Conflicts group will require a Date of Loss.

        Accident Location
            •  Can be entered on up to three lines (Retention Agreement is limited to three lines).
            •  State must be abbreviated.
            •  City names that have St. v. Saint, or Ft. vs. Fort, use the abbreviation.
            •  Include zip code if available.
            •  Do not include party names in the Address of Loss field.  This field is strictly for the loss location.
            •  Example:
                   o  7001 Buffalo Parkway, Suite 1A
                       Columbus, OH 43229
            •  This field has a spell check function. Use spell check before moving onto the next step (circled in red on the next
               illustration).

        Description of Loss
            •  Describe the loss.
            •  Do not include party names in this field.
                   o  To explain a driver of a vehicle and the vehicle type, enter this in the "How Party Relates" field under
                       "Related Parties."
            •  VIN numbers and insurance companies should be added in this field, identified by V1, V2, V3, which is related to
               the How Party Relates field.
            •  This field has a spell check function.  Use spell check before moving onto the next step (circled in red on the next
               illustration).

        Scope of Project
            •  What S-E-A has been hired to do.
            •  Examples:  Origin & Cause, Assess Water Damage.

        Practice Group
            •  This field is a dropdown.
            •  Once familiar with the terms, type in the first letter of the word and the word will auto fill.

        Tracking Type
            •  This field is a dropdown.
            •  If the matter needs a conflict check, choose Standard.
                   o  If Standard is selected, Related Parties becomes a required field.
            •  Do not enter Related Parties for Deposition or Testing matters.
