Firearm Theft/Loss Report

 This report must be completed for the theft/loss of firearms and must be completed within 48 hours of

discovery.

 There is a toll free number on the back of the form to call to process the report.

 Make a copy of the report and the original is forwarded to the National Tracing Center and the

address is located on the back of the form.

 The copy of the form must be retained in your store as part of the stores permanent records.

 Contact your Supervisor and Firearms Compliance Officer for instruction on processing the firearm

from your gun book record.

 If the firearm(s) is later discovered found you should advise ATF that firearm(s) has been located.

 Ensure the recovered firearm is re-entered into the Gun Book.
