AN ADMINISTRATIVE REGULATION
GOVERNING THE ISSUANCE AND USAGE
OF EMPLOYEE IDENTIFICATION BADGES AND ACCESS CARDS
AND SUPERSEDING AR-02-01
Purpose: The City of Farmington believes strongly in providing a safe work environment for all City
employees and exceptional customer s~rvlce to the citizens we serve. To facilitate this commitment, the
City has revised the ID Badge policy for all City employees. This City of Farmington ID Badge policy will
allow customers and other employees to quickly Identify individuals as City employees.
Polley: All employees will be issued and must clearly display their Employee identification badges and
lanyards or armbands as outlined in the Proper Wear section of this policy. ID badges and facility access
cards will be authorized by the Department Head and issued through the Human Resources
Department. Each Identification badge will contain the employee's name, department, position title,
photograph, and contain a valid telephone number for verification purposes. Identification badges and
access cards will automatically expire every four years and the expiration date will also be printed on the
badge or tag. Employees are to use their badge only in the course of their employment, and to take
appropriate measures to see that their badges are not accessible for use by unauthorized persons.
Lost, Stolen or Damaged ID badge: Should an Identification badge and/or access card be lost, stolen or
damaged the employee is to Immediately report such loss to the Department Head. If an employee
forgets their ID Badge they wlll be required to retrieve the badge or go to HR for a temporary card using
their own personal time. Each employee may receive one free replacement badge every 4 years a card is
issued. A charge of $5.00 shall be levied upon the employee if they require a new badge after receiving
their one free replacement within the four year period, not including replacement for normal wear and
tear.
Proper wear of ID badge: Employees must wear and clearly display the photo identification badges as
provided by the City by either:
A. City issued lanyard with a safety breakaway
B. City issued belt clip- (should not be covered up by other clothing)
C. City issued arm Band
D. Other City Issued badge mount or display as approved by the department head.
Each department will be responsible for ensuring that badges are worn as required.
Upon approval by the Department Director or his/her designee, individual departments may exempt
their employees from wearing their ID badge for safety or while out in the field if those employees are
required to wear a uniform that clearly identifies them as a City employee. Those employees, however,
are required to carry their badge while in the field in order to provide further verification.
