AN ADMINISTRATIVE REGULATION GOVERNING THE HEALTH AND WELLNESS PROGRAM AT
CITY-OWNED RECREATIONAL FACILITIES
1. Purpose: Provide opportunities for employees to engage in active and healthy lifestyles that improve
morale, mental, and physical health. The intent is to provide opportunities for the employees to receive
discounted services as a benefit of employment and component of health and well-being in the
workforce.
2. Scope/Application: This policy applies to all current employees (Including part time, temporary,
seasonal and regular classifications). Participation in this program is available to employees upon their
date of hire.
The program is for the Employee and immediate family. The city employee and members of the
immediate family are eligible. Immediate family is defined as the employee, spouse, and children that
could be claimed as a dependent. The immediate family member(s) must purchase the benefit at the
same time as the employee.
In order to receive the benefit, the employee must provide a current City of Farmington identification
badge at the time of purchase. (For temporary or seasonal employees who do not receive a City of
Farmington ID Badge as part of their normal employment, a most recent pay stub will suffice as
identification)
Benefit discounts may be reviewed periodically and updated by the City Manager.
Regular full-time employees may use the payroll deduction for annual passes at Pinon Hills Golf Course,
Civitan Golf Course, Farmington Aquatic Facility, Lions Pool, or the Brookside Bay Waterpark. In the
event employment with the city ends, the pass on payroll deduction will be suspended until full
payment in made.
Bona fide PERA retirees who previously worked for the City will be eligible for this benefit. Retirees will
need to obtain a Retired Employee ID badge from the City of Farmington Human Resources department
and present to the facility as proof of their eligibility for the program.
