Benefits provided by Molzen Corbin, such as health/dental insurance and
vacation/sick leave accruals, will continue while the employee is receiving a
paycheck from Molzen Corbin (subject to meeting the requirements of the Family
and Medical Leave Act). Once the employee has exhausted accrued leave and is no
longer receiving a paycheck from Molzen Corbin, all Company-provided benefits
will cease. If and when the employee returns to full-time active employment, benefits
will begin again.
G. FAMILY AND MEDICAL LEAVE ACT
1. Eligibility for Leave. Molzen Corbin will comply with the Family and
Medical Leave Act of 1993 (FMLA). In general, the Act allows up to
12 weeks of unpaid job-protected leave. All staff members who have at least
12 months of service (whether consecutive or not), and who have worked
1,250 or more hours during the preceding 12 months, and who at the time of
request work at a company location where 50 or more employees work within
75 miles, may be eligible for unpaid Family and Medical Leave.
2. Required Notice and Medical Certification. If the necessity for the leave is
foreseeable, an employee must provide Molzen Corbin with thirty (30) days’
advance written notice of a request for leave, but in any case, notice is
requested as soon as such notice is practicable (i.e., within one or two
business days when the need for leave becomes known to the employee).
An employee must give Molzen Corbin verbal or written notice of FMLA
impending leave as soon as practicable.

the anticipated leave date; and
the anticipated duration of the leave.
Appropriate documentation may be requested to verify the reasons for the
leave. Any requested leave based on a serious health condition, whether it
involves the employee or a family member, may require an appropriate
medical certification by a licensed health care provider. In all cases of leave
for medical reasons, Molzen Corbin reserves the right to request a second
medical opinion at Company expense and further medical opinion, where
appropriate. Periodic recertification also may be required for requested
extensions of medical leave, absences which exceed thirty (30) days, and
other appropriate circumstances.
