Voluntary Term Life/AD&D
Employee minimum benefit of $10,000.
Employee may choose increments of $10,000 for totals of
$20,000, $50,000, $70,000, $100,000, not to exceed 5
times annual salary. Guaranteed Issue is 5x annual salary or $100,000, if
enrolled when first eligible.
Term Life-reduces as you age with first reduction at age 70
Employee’s spouse may elect coverage in increments of $5,000; may chose
coverage amounts of $5,000, $10,000, $15,000, $25,000. Guarantee issue
amount is 100% of employee’s benefit or $25,000.
Employee may also elect dependent child coverage in increments of
$1,000, for a minimum of $2,000 up to $10,000 maximum. Guarantee issue
amount is $10,000.

Worksite Benefits
Hospital Confinement Indemnity Eases the financial burden of hospital visits by
providing cash benefits to help with any out-of-pocket costs not covered by your
major medical insurance.
Accident Helps provide financial stability for everyday expenses and medical
treatment if a covered accident occurs.
Cancer/Specified-Disease Helps cover expenses from initial diagnosis of a covered
cancer, through treatment and follow up visits.
Critical Illness (Specified Health Event) Helps with the costs of treatment if you
experience a covered health event, such as a heart attack, stroke, or paralysis.
Aflac Plus Rider Pays a lump sum benefit amount along with additional benefits
when you are diagnosed with a covered health event.
Juvenile Life Provides term or whole life insurance for your children or grand-
children ages 14 days through 17 years.
