Health Savings Account


                            You have the option to open a Health Savings Account through Allegacy
               Federal Credit Union if you choose to enroll in the Core Medical Plan through P&A.  A
               Health Savings Account provides you with a tax advantaged way to start saving for
               future medical expenses.  P&A contributes $500 contribution per plan year for employee
               only and $1,000 for employees with dependent coverage.  You may also contribute
               funds pre-tax.  The IRS limits the amount of contributions per year as follows: $3,400 for
               employee only and $6,750 for employees with dependent coverage.
               The advantages to opening an HSA with Allegacy include:
                     No fees
                     Minimum of $5 to open account
                     Easy Debit Card Access to your HSA funds
               Please note that required application materials must be completed in order to open an
               HSA.  These materials may be obtained from P&A’s Human Resources Department.


                 Flex Spending Account




               Provided by WageWorks
               Employees can contribute a maximum of $2,500, to be used for qualified health expenses such
               as medical copays, deductibles, dental and vision expenses.
               Debit cards will be provided to all enrolled employees at no cost.
               This plan allows employees to roll over $500 into the next year.

               Employees who have both the FSA and HSA account, the FSA can only be used for non-medical
               expenses, like dental and vision.

               Care24 Services EAP Provided by UHC
               Care24 services offer you access to a wide range of health and well-being information seven
               days a week, 24 hours a day.  Call the number on your member ID card, or the National Relay
               Center at 1-800-828-1120, you will be connected to experienced professionals: Registered
               Nurses, Master’s-level counselors, legal and financial professionals, and community resources.
               Assistance is available for childhood illnesses, minor illnesses and injuries, medication safety,
               relationship worries, choosing appropriate medical care, stress and anxiety, coping with grief
               and loss, personal legal and financial issues, self-care information, help finding a doctor,
               information on medications, and general health information.

                                    772-1990, (between 5pm-9am EST) 1-919-796-4102.  Provide company
                                    code (Code: P&A180) to counselor upon connecting in order to authorize
                                    services.  There is no charge.
