Benefit Costs
Effective January 1, 2018 – December 31, 2018
Full-time employees working a minimum of 30 hours per week are eligible for benefits on the first of the month following 30 days of
employment. If an employee fully participates in the plans listed below, Zumba Fitness will be contributing over $4,500 annually toward an
employee’s benefits.
Medical Plan – Cigna
Election Cost
Employee only $49.02 per payroll
Employee + spouse $170.44 per payroll
Employee + child(ren) $152.95 per payroll
Employee + family $234.89 per payroll
Health Reimbursement Account – Cigna
For employee only: Up to $2,000 reimbursement to cover out-of-pocket expenditures between $2,000.01 and $4,000 per year. After
this, plan kicks in at 100%.
For employee plus dependents (family): Up to $4,000 reimbursement to cover out-of-pocket expenditures between $4,000.01 and
$8,000 per year. After this, the plan kicks in at 100%.
