        Life & Disability Insurance




        Life and Accidental Death & Dismemberment (AD&D)

        Life insurance is an important part of your financial security, especially if you support your family.
        The company provides basic life and accidental death and dismemberment insurance to all eligible employees at no cost. Coverage is automatic.
        Accidental Death & Dismemberment (AD&D) Insurance provides a benefit in the event of your accidental death or dismemberment.

        Voluntary Term Life Insurance

        You may purchase Voluntary Life Insurance for yourself in units of $10,000. The maximum for any employee is $500,000. The guaranteed
        coverage amount for you is $200,000 without providing evidence of insurability (EOI).

        You may also elect the following coverage for your spouse and children:
        •  Your Spouse: You may select coverage for your spouse in units of $5,000 to a maximum of $500,000. Spouse can not elect more
            than 100% of the employee amount. The cost of coverage will be based on your age. The guaranteed coverage amount for your
            spouse is $30,000.
        •  Your Unmarried, Dependent Child: You may select coverage for your unmarried, dependent children in the amount of $10,000.
            The maximum benefit for children under six months is $500. The guaranteed coverage amount for your Child(ren) is $10,000.

        Disability Insurance Coverage
        Disability insurance provides income replacement should you become disabled and unable to work due to a non-work related illness or
        injury. The company provides eligible employees with short-term disability income benefits at no cost as shown below. Coverage is automatic
        and you do not need to enroll. Zumba® also offers long-term disability coverage as a voluntary benefit at your own cost.



                                                     Coverage and Benefits

         SHORT-TERM DISABILITY
         Covers 66.67% of your weekly earnings, to a $2,500 maximum. Benefits begin on: Accident: 8th day,  Illness: 8th day, Hospitalization:
         1st day  for up to 13 weeks. Zumba covers the elimination period before benefits begin.
         VOLUNTARY LONG-TERM DISABILITY
         Covers 60% of your base annual earnings, to a maximum of $10,000 monthly. Benefit begins after 3 months of disability to age 65.

        •  Car payments & maintenance
        •  Mortgage or Rent






             It’s important to periodically update your beneficiary in Ultipro in order to make sure the right people receive your assets.
