required based on the type of change, i.e. name change requires a copy of your social security
card showing the new name or other appropriate legal document.
Because the information in your personnel file is personal by nature, we keep the file as
confidential as possible. We allow access to your file only on a need-to-know basis.
You are responsible for contacting the Human Resource Service Team to discuss benefit
continuation and conversion rights in the event of a divorce, legal separation, or a dependent
ceasing to be eligible for coverage. All changes must be in writing and signed by you. Contact
your Human Resource Service Team to obtain any required change form(s).
SEPARATION FROM EMPLOYMENT
As an at-will employee you are free to terminate your employment relationship with us at any
time, for any reason. However, we request that you provide two (2) weeks’ written notice.
Failure to do so may affect future consideration for re-hire.
Once you submit a notice of resignation, time-off will generally not be approved or permitted
during the final two-week period of employment. An exit interview will ordinarily be scheduled
on or close to your last day of employment.
EMPLOYMENT AND INCOME VERIFICATION
All requests for personnel information concerning applicants, current employees, or former
employees from outside sources must be directed to the Human Resource Service Team.
Generally, the Human Resource Service Team will only release information in response to
written requests after obtaining the written consent of the individual who is the subject of the
inquiry. The Human Resource Department may confirm employment dates, positions held,
salary, and reason for leaving without first obtaining the consent of the individual involved.
We will comply with all subpoenas for information, documentation, etc. We will also supply
information, documentation, etc. as requested by federal, state, and local authorities without
first obtaining your consent.
UNEMPLOYMENT COMPENSATION INSURANCE
Unemployment compensation insurance provides benefits to eligible workers who become
unemployed through no fault of their own, and meet certain other eligibility requirements.
Monetary payments are provided to eligible workers for a given period of time or until they find
a new job.
