warning will normally cease to be “live” after six (6) months, that is, it

                       7.5 First Written Warning

7.5.1 If the employee’s performance, conduct or attendance does not
sufficiently improve after the verbal warning or there is a more

may be necessary to issue a first written warning. The written warning
will outline the nature of the default, the action required to remedy it,
the specified time period within which improvement should be made
and the consequences of the failure to do so, and the employee’s
                                     right of appeal.

7.5.2 The written warning will be confirmed to the employee in writing and
a copy will be placed in the employee’s personnel file for a period of
12 months, after which time it will be disregarded for disciplinary
purposes, subject to satisfactory conduct and performance.

or performance or where conduct previously complained of re-occurs,
a final written warning will be issued. This will outline the nature of
the offence, the action required to remedy it and the consequences of
the failure to do so and the employee’s right of appeal. It will also
detail that any recurrence of the offence or other serious misconduct
within the allocated improvement period may result in dismissal.

7.6.2 Where an employee is found to have committed an act of Serious
Misconduct they will receive a final written warning and any
subsequent offence will result in dismissal.

7.6.3 The final written warning will be confirmed in writing to the employee
and a copy will be retained in the employee’s personnel file for a
