What is “closing the loop” or providing the final

disposition on ECIN?

 Referrers base their discharge plans on VNSNY’s ability to
accept a patient for homecare, therefore it is critical that a
“final disposition” is provided, noting whether or not each
referral is accepted.

 Under the referral status field within ECIN, the Intake team
can select from a dropdown whether VNSNY is accepting a
referral, either:
 (“Yes, willing to accept patient”)
or
 (“No, Unable to accept patient”).
