                   •  To participate in safety planning, including temporary or permanent relocation, to increase
                       the employee’s or the employee’s family or household member’s safety from domestic or
                       sexual violence;
                   •  To seek legal assistance to ensure the employee’s or the employee’s family or household
                       member’s health and safety, including participating in court proceedings related to the
                       violence.

               Employees eligible for leave under this policy may take up to four (4) weeks of unpaid leave during
               each calendar year. Eligible employees intending to take leave under this Act shall provide Lupin
               with at least forty-eight (48) hours advance notice, unless providing notice is not practicable. Lupin
               may require employees to provide a certification of their leave by providing a statement from any
               of the following: a victim services organization; an attorney; a member of the clergy; or a medical
               or other professional from whom the employee or the employee’s family or household member
               sought assistance, or other corroborating evidence of the employee’s need for leave.

               Payment of Accrued, Unused PTO Upon Termination

               Upon termination of employment, Illinois employees are entitled to payment for all accrued, but
               unused PTO time. The  payment will be made at the time of and in  the same manner as the
               employee’s final pay.
