Prior to taking time off for purposes of vacation, personal leave, military or jury
duty, or other planned absence, an Employee Leave Request Form, available
from a member of the Leadership Team, should be submitted to and approved by
your manager.
Employee Leave Requests should be submitted at least two weeks prior to the
scheduled leave date, unless the request is due to an unexpected emergency.
The circumstances of the emergency (excluding private medical information)
should be shared with the General Manager.
If you are absent due to illness of yourself or a family member for four or more
workdays, we will require you to produce a doctor's certificate when you return to
work.
Employees who miss four or more days due to a medical condition are required
to bring a doctor’s release to return to work. In addition, to return to work from an
accident or medical leave, all employees must present a doctor’s certification
permitting the return to work.
Any employee who fails to return to work at the expiration of a personal leave of
absence will be deemed to have abandoned their job, unless Amy’s Drive Thru is
notified of a reason, satisfactory to management, for not returning to work at the
end of the leave of absence.
IMPORTANT: Although attendance is important, we do not want you to come to
work if you are ill and you have symptoms associated with an acute
gastrointestinal illness (such as vomiting, diarrhea, abdominal cramps, fever)
and/or are known to be infected with a communicable disease transferred
through food (Salmonella, Shigella, Norovirus, etc..), It is critical that you not
work at Amy’s Drive Thru until you are determined to be free of the
illness/disease and all symptoms are gone (California Retail Food Code,
Employee Health 113949). All employees must report any illness that can be
transferred through food. Please speak to your Manager if you have any
questions about this policy.
TARDINESS
Employees must be prepared to start work promptly at their scheduled shift start time.
Your scheduled time is the time you are expected to be on the job, not arrive at the
Restaurant. Repeated tardiness is grounds for disciplinary action up to and including
