3.  Once the desired benefits dashboard has loaded, please follow the
          screen prompts to complete benefit enrollment selections. Click “Read
          More” to learn more about each benefit in UltiPro.

                                                                                             Midyear Change
                                                                                             A qualified midyear change that
                                                                                             allows you to make benefit
                                                                                             changes include the following:
                                                                                             •   Marriage, divorce,
                                                                                                annulment or legal
                                                                                                separation
                                                                                             •   Birth or adoption of a child

                                                                                             •   Change in child’s
                                                                                                dependent status
      4.  Benefit enrollments must be submitted     after all selections have
