Changing Your Elections Mid-Year

                                            While the benefits choices you make during annual enrollment remain effective
                                            for the entire next year, there are special circumstances when changes are

                                            allowed, called qualified mid-year election changes. Certain benefit changes may
                                            be made if  they are consistent with the event. For example, if  you have a child,
                                            they may be added to your medical coverage and/or you can add dependent life
                                            insurance.

                                            If  you experience a qualified change, contact benefits administration as soon as
                                            possible (but no longer than 30 days) to ensure you submit your benefit changes
                                            within the required time period.

                                           Paying for Coverage


                                           You and GESMN share in the cost of  your benefits, with GESMN paying the
                                           majority. You pay your share through payroll deductions. When you pay premiums
                                           pre-tax, you don’t pay federal, state (in most states), Medicare or Social Security
                                           taxes on your contributions. Therefore, a $100 contribution might actually cost you
                                           $80  with the tax savings, or even less, depending on your tax bracket.

                                           Other benefits, like flexible spending accounts and voluntary life insurance, are
                                           funded by you, with the cost of  the plan administration covered by the  company.


                                           Who’s Eligible for Benefits?


                                           Eligible Employees

                                           Employees working at least 30 hours or more a week are eligible to participate in  all
                                           benefit plans.

                                           Employees averaging between 20 – 29 hours per week are eligible to participate in
                                           certain voluntary (employee paid) benefits as outlined in this guide (i.e. dental,
                                           vision, Flexible Spending Account, voluntary life, hospital indemnity, critical illness

                                           and accident).

                                           Employees averaging less than 20 hours per week are not eligible for GESMN
                                           benefits.

                                           Eligible Dependents

                                           Your legal spouse, qualified domestic partner and dependent children up to age
                                           26* are also eligible to  participate.
                                            *    Dependent children with disabilities can be covered beyond age 26 if they meet certain criteria. See
                                              your  human resources representative for more details.
