DISABILITY ANd LIFE INSURANCE






            DISABILITY INSURANCE
            Disability insurance provides income replacement should you become disabled and unable
            to work due to a non-work-related illness or injury. The company provides disability coverage
            through Cigna at no cost as shown below. Coverage is automatic; you do not need to enroll.


               COVERAGE                                       ELIGIBLE EXPENSES

             Short-Term     •   60% of your weekly salary, to a maximum of $2,000 per week for the first 13 weeks of a disability after the
             Disability      one-week waiting period.

             Long-Term      •   60% of your base salary, to a maximum of $10,000 per month if you are disabled and are unable to work
             Disability      for more than 90 days.
                            •   Benefits are offset with other sources of income, such as Social Security and Workers’ Compensation.



            LIFE AND ACCIDENTAL DEATH AND DISMEMBERMENT (AD&D) INSURANCE
            Life insurance is an important part of your financial wellbeing, especially if others depend
            on you for support. Life Insurance pays benefits if you die from any cause. AD&D benefits are
            paid if your death is accidental or if you are severely injured due to an accident and suffer a
            loss specified in the policy such as loss of a limb or eyesight.

            The company provides Basic Life and AD&D insurance at no cost equal to two times your
            base annual salary, up to a maximum of $150,000. Coverage is automatic; you do not need
            to enroll.


            SUPPLEMENTAL LIFE INSURANCE
            You may choose to purchase additional Cigna Life coverage for yourself and your dependents
            at affordable group rates. Rates are based on age and the coverage level chosen.

            All eligible employees can opt in or increase existing employee or spouse coverage up to the
            guarantee issue amounts without providing evidence of insurability.
