Voluntary Life Coverage


        Voluntary Life insurance for you, your spouse/domestic partner and children can help protect your family during difficult times.
        Eligible employees may purchase Voluntary Life for themselves and their family.

         COVERAGE FOR               COVERAGE AVAILABLE
                                    1–4x annual salary, round to the next higher $1,000, to a maximum of $750,000.
         Employee
                                    Guaranteed Issue of $300,000*.
                                    Increments of $5,000 up to $100,000 – not to exceed 50% of employee coverage.
         Spouse/Domestic Partner
                                    Guaranteed Issue of $20,000*.
         Child(ren)                 Flat $10,000 per child. $500 for children – birth to 6 months.
         Employee and Spouse/
         Domestic Partner           Benefits will be reduced as follows: 65% at age 70; 45% at age 75; 30% at age 80; 20% at age 85.
