Q. I ride my bike to work. Does Rubrik provide a subsidy to bicyclists?
A. Yes. In lieu of the Commuter benefit, the Bicycle Benefit allows employees who use a bicycle as their primary
means of commuting to work to receive a subsidy of $20 per month. The $20 per month subsidy can then
be used toward expenses related to your bicycle maintenance and commute. Examples of eligible expenses
include the purchase of a new bicycle, helmets and other protective gear, and bicycle maintenance
and storage.
Q. What is the difference between Edenred and other commuting options provided at HQ?
A. Edenred is a platform that allows you to use pre‑tax funds to pay for transit or commuter parking expenses
not covered or provided by Rubrik HQ (e.g. BART passes).
Leave of Absence
Q. When and how should I report a Leave of Absence (LOA)?
A. If you will be missing work for longer than 2 weeks (consecutively or intermittently) due to a serious illness
(your own or a family member’s), disability, military or maternity/paternity leave, you should contact the
Rubrik Benefits Team to determine if a leave of absence would best support you. To learn more about the
Rubrik leave of absence policy and processes, please watch this video. If you have questions about leave of
absence, please email benefits@rubrik.com.
Q. How do I contact TRISTAR (Rubrik’s Leave of Absence Administrator)?
A. You may visit ess‑absencetracker.com or call an Intake Specialist at 844‑702‑2352 to report your leave or
check on the status of your leave. TRISTAR is available to assist you 24/7. Be sure to loop in your manager as
well as the Rubrik Benefits Team with your official leave of absence dates.
Wellness Reimbursement
Q. What is Rubrik’s Wellness Reimbursement program?
A. Rubrik offers a wellness reimbursement stipend of $50 a month per employee (accruing to $600 annually)
to help employees maintain a well‑rounded, healthy lifestyle. The stipend can be used to support a variety
of eligible wellness activities and items. For 2022, Wellness Reimbursement will be administered by Forma.
The Forma benefit is accessible through Okta.
Q. How do I contact Forma?
A. Forma’s friendly Member Experience Team is available Monday through Friday during regular business
hours. They may be reached via email at support@joinforma.com or via In‑App Chat.
To begin a conversation via In‑App Chat, follow these steps:
Once logged in to your Forma account, you’ll notice a circular button hovering in the lower right‑hand corner.
Click on the button to bring up a list of options.

Q. What items are considered eligible under the Wellness program policy?
A. A guideline of eligible items is available under the program policy. Please refer to the policy listed in your
Forma account (under Claims ‑‑‑ > “Read Rubrik’s Program Policy”). Or, contact our Member Experience
