                   Fourth generation

                                           Cefepime, Cefpirome


                       •  Extended spectrum agents with similar activity against Gram-positive
                          organisms as first generation cephalosporins

                       •  Excellent activity against Enterobacteriaceae and Pseudomonas
                          aeruginosa
                       •  Anti-pseudomonal activity = Ceftazidime, Piperacillin/tazobactam
                       •  Greater resistance to beta-lactamases than the third generation

                          cephalosporins.
                       •  Can cross the blood–brain barrier and are effective in meningitis
                       •  limited activity against anaerobes


                       •  Cefepime is used as Empiric therapy of infection (ventilator-associated

                          pneumonia)

                       ❑ 4th generation cephalosporins are drugs of last resort for serious
                          infections
