c.  The  Credit  Union’s  retirement  plan  will  suspend  upon  active  military  and  resume,  at  the
                     employee’s election.

                  d.  Voluntary supplemental life/AD&D insurance will terminate the day the employee becomes
                     active military.

                  e.  Health benefit will be extended at 100% cost to employee up to 12 months.

                  f.  Annual  and  sick  leave  accrual  will  be  suspended  on  the  last  pay  date  worked  and  will  be
                     resumed upon re-employment.

              Employees  are  requested  to  notify  their  Supervisors  as  soon  as  they  are  aware  of  the  military
              obligation. Human Resources will, upon the employee’s return, request that the employee provide
              the  Credit  Union  with  military  discharge  documentation  to  establish  the  timeliness  for  re-
              employment,  the  duration  of  the  military  service,  and  the  honorable  discharge  from  the  military
              service.


              Questions regarding the company’s military leave policy are in accordance with USERRA, applicable
              state and federal laws and continuation of benefits should be referred to Human Resources.

              BEREAVEMENT LEAVE

              Bereavement leave with pay is provided by the Credit Union in the case of a death in the immediate
              family (spouse, parents, grandparents, children, brothers, and sisters) and shall be granted by the

              employee’s Supervisor upon proper notification. Such paid leave will typically be from one to three
              days depending on the circumstances such as closeness of relationship and/or distance traveled to a
              funeral or other observance.

              Police  FCU understands  that  there  may  be  unforeseen  circumstances  surrounding  the  death  of  a
              family member that may require extensions. Such extensions will be reviewed on a case-by-case basis
              by the employee’s Supervisor.  If an extension is granted beyond the first three days, the time away
              from work will be charged to paid vacation leave.


              ADMINISTRATIVE LEAVE

              Reasons for use of administrative leave may include, but are not limited to separating an employee
              from work while an incident or concern is investigated or evaluated. Additionally, administrative leave
              may also be used infrequently on a case-by-case basis as an incentive by a Supervisor, and/or coded
              on  timesheets  as  it  relates  to  weather  emergencies  as  announced  by  the  President/CEO.

              Administrative leave may be with or without pay.

              NO CALL/NO SHOW

              Not reporting to work and not calling to report the absence is a no-call/no-show and is a serious
              matter. No call/no show for two days may result in disciplinary action and may be considered job
              abandonment and deemed voluntary employment termination.
